Community driven ICF/ID

Hills & Dales is proud to offer services to 59 residents through our Intermediate Care Facility. We focus our efforts on supporting medically-fragile children and children with challenging behaviors due to an autism or other diagnoses.

While we work to make this service most available to children, we review the circumstances of every applicant, even adults. From day one, we support residents in planning transition when appropriate to more independent service opportunities. To date we have supported more than 40 of our residents in achieving health and behavioral stabilization enabling a life in a a community setting.

    Supporting the medical and health needs of residents every day of the year.

  • Integrative approach to Behavioral supports

    Staff receive specialized training to promote and maintain safety for persons supported.

  • supporting children, young adults, and medically fragile individuals

    Each resident receives individualized support and services to maximize his or her skills and abilities.

  • Internal Adaptive equipment technician

    Custom adaptive or mobility equipment created on-site.

  • Vocational Skills Training Center

    Adults living at the Residential Center learn various life skills through the Life Training Center (LTC), which is located at the Community Center in Dubuque. This program makes Bogey Tales Dog Treats that are sold in various locations, with all proceeds supporting the people who make them, and complete various other projects.
