HCBS Program Coordinator

Full-Time Salaried Position 

Schedule: Includes Rotating On-Call Schedule including weekends, some evening and weekend hours may required.

The Home and Community-Based Services Supervisor is responsible for the supervision and day-to-day operations of the community-based services program. This job will direct the work and provide supervision and leadership of assigned personal assistants through mentoring, training and development of a cohesive team. This position is responsible to the department director in developing and providing strategic business direction of the community-based services program, and thus, requires a high level of organization, decisiveness and the ability to multi-task.

• A Bachelor’s degree in a Human Services field is preferred, however an Associate of Arts degree with one-year experience in a direct care setting will be considered, or a minimum of two years’ experience in a direct care facility
• A minimum of 1-2 years supervisory experience required
• Free from convictions or prior employment history of child or resident abuse or dependent adult abuse
• Ability and desire to work with children and/or adults with mental and physical disabilities
• Valid driver’s license*

Lead HCBS Personal Assistant

Full Time, 2nd Shift

The Personal Assistant Lead (PA) is responsible for helping to build meaningful lives for the adults we serve and for enhancing the quality of their lives. This is accomplished through the implementation of person centered planning, individual goals, and self-assuring experiences in a manner that is consistent with the policies and procedures of Hills and Dales.  

specific duties and responsibilities

  1. Provides personal support and active engagement of clients to maintain a therapeutic, growth producing relationship.
  2. Provides physical supports for clients.
  3. Assists, supports and accompany clients for appointments and various activities such as community events, spiritual enrichment activities, church services, shopping etc.
  4. Promotes the dignity of the adults we serve.
  5. Ensures the safety and health of self, co-workers and clients by following all safety procedures.
  6. Provides support to  Supervisor with the following duties/tasks:
  7. Assures the confidentiality of all data, including client, employee and operations data.  Complies with HIPAA privacy and security standards as identified in the agency policy.
  8. Responds to directives as required and in a timely manner.
  9. Assists co-workers as required.
  10. Attends meetings, Individual Program Plan meetings, and scheduled in-service meetings as required and actively participates.
  11. Follows and maintains established department and agency policies, procedures, regulatory requirements, quality, safety, environmental, and infection control standards.
  12. Attendance and punctuality as per schedule, and work ethic such that the job performance meets or exceeds expectations.
  13. Supports supervisor in evaluating and monitoring employee performance.  In the event follow-up is required, will issue staff performance documents to employees with guidance; prior authorization is required from immediate supervisor or designee for those performance situations that may lead to further discipline.  Positive feedback may be given spontaneously, without authorization from supervisor or designee.   Will also sit in as a witness to staff counseling conducted by the supervisor.
  14. Performs other related duties as requested by supervisor.
  15. Assist with scheduling staff coverage (shifts, appts, etc.)

EDUCATION, Experience and TRAINING

  1. Free of a conviction or prior employment history of child or resident abuse or dependent adult abuse.
  2. Eighteen years of age or older.
  3. High school diploma or equivalent, required.
  4. Valid driver’s license with proof of insurance, if required.
  5. Must become certified in CPR/First Aide within the first six months in position and maintain certification thereafter.
  6. Must successfully complete MANDT & med manager training and maintain certification.
  7. One year of direct support experience in a formal support provision setting and/or CNA preferred and at least three months experience with HCBS Waiver Services for Supported Community Living.

Personal Assistant, Full-time

Starting wage is $12.00. 

Additional pay for 1 or more years work experience in a HCBS &/or college related education for AA or BS/BA.  Shift and weekend differentials also available.

Must be 18 years of age with a current, valid driver’s license.

Offer of employment contingent upon successful completion of criminal background checks and drug screening. EOE

First shift -Hours range from 6:00am – 3:30pm and may include hours at the Hills & Dales Day Habilitation Program.  Every other weekend rotations are included in every schedule and include an additional weekend pay differential of $4/hour.

Second shift -Hours vary from 3:30pm-4:00pm to 9:00pm–11:00pm.  Every other weekend rotations are included in every schedule and include an additional weekend pay differential of $4/hour.

Third shift -Hours vary from 10:00pm–11:00pm to 7:00am–9:00am.  Third shift hours consist of sleep, (yes, get paid to sleep!) time/pay.  Every other weekend rotations are included in every schedule and include an additional weekend pay differential of $4/hour.


Personal Assistant, Part-time

Starting wage is $12.00. 

Additional pay for 1 or more years work experience in a HCBS &/or college related education for AA or BS/BA.  Shift and weekend differentials also available.

Must be 18 years of age with a current, valid driver’s license.

Part-time positions are available on various shifts and range from 12-36 hours per pay period. Every other weekend rotations are included in every schedule and include an additional weekend pay differential of $4/hour.


Personal Assistant III

Starting wage is $12.00 – $14.50 based on experience.

Additional pay for 1 or more years work experience in a HCBS &/or college related education for AA or BS/BA.  Shift and weekend differentials also available.

Personal Assistant III’s positions specializes in behavioral supports to support young, adult males within a community setting. Every other weekend rotations are included in every schedule and include an additional weekend pay differential of $4/hr.

Personal Assistant III, Full-time
Positions are available on various shifts.

Personal Assistant III, Part-time
Positions are available on various shifts, ranging from 16-37 hours per pay period.

Requirements:

  • High school diploma or equivalent, required. AA or BA in a related field is preferred.
  • Valid driver’s license and proof of insurance, in accordance with company policy
  • Two years of direct support experience in a behavioral health setting is preferred
  • Medication Manager Certification and CPR & First Aid Certifications are preferred, however training will be provided if you are not currently certified.

In-Home Services

Personal Assistant, Part-time

Flexible scheduling with hours ranging from 3 – 7 hours per pay period.

Additional pay for 1 or more years work experience in a HCBS &/or college related education for AA or BS/BA.  Shift and weekend differentials also available.

Must be 18 years of age with a current, valid driver’s license.
