HCBS Program Coordinator

Position located in Dubuque, IA

Full-Time Exempt, Salaried Position HCBS In-Home
Schedule: Monday – Friday 8:00am – 5:00 pm

On-Call Schedule including weekends, some evening and weekend hours may required.

The Home and Community-Based Services Supervisor is responsible for the supervision and day-to-day operations of the community-based services program. This job will direct the work and provide supervision and leadership of assigned personal assistants through mentoring, training and development of a cohesive team. This position is responsible to the department director in developing and providing strategic business direction of the community-based services program, and thus, requires a high level of organization, decisiveness and the ability to multi-task.

  • A Bachelor’s degree in a Human Services field is preferred, however an Associate of Arts degree with one-year experience in a direct care setting will be considered, or a minimum of two years’ experience in a direct care facility
  • A minimum of 1-2 years supervisory experience required
  • Free from convictions or prior employment history of child or resident abuse or dependent adult abuse
  • Ability and desire to work with children and/or adults with mental and physical disabilities
  • Valid driver’s license and insurance required

Starting wage of $42,744 depending on experience.

Lead Personal Assistant

Starting wage is $16.50 plus an additional $1.50 after 90 days.
$2/hr 2nd shift differential
$3/hr differential for weekend shifts!

The Personal Assistant Lead (PA) is responsible for helping to build meaningful lives for the adults we serve and for enhancing the quality of their lives. This is accomplished through the implementation of person centered planning, individual goals, and self-assuring experiences in a manner that is consistent with the policies and procedures of Hills and Dales.

Personal Assistant

This is accomplished through the implementation of person centered planning, individual goals, promoting development of life skills, community integration & self-assuring experiences in a manner that is consistent with the policies and procedures of Hills and Dales

Must be 18 years of age with a current, valid driver’s license. Applicants must qualify as an approved driver; use of personal vehicle with minimum insurance requirements of $ 25,000.00 bodily injury liability (found on Declarations page of your policy).

 


Personal Assistant III

NEW Starting wage is $16.75 – $18.25 based on experience.
$2/HR 2ND shift differential
$3/HR differential for weekend shifts!

Personal Assistant III’s positions specializes in behavioral supports to support adults with intellectual disabilities within a community setting. The Personal Assistant III is responsible for helping to build meaningful lives for the individuals we serve and for enhancing the quality of their lives. This is accomplished through the implementation of active engagement, individual programs, and self-assuring experiences in a manner that is consistent with the policies, procedures and mission statement & values of Hills and Dales.

Every other weekend rotations are included in every schedule and include an additional weekend pay differential of $3/hr.

 

Requirements:

  • High school diploma or equivalent, required. AA or BA in a related field is preferred.
  • Valid driver’s license.  Applicants must qualify as an approved driver; use of personal vehicle with minimum insurance requirements of $ 25,000.00 bodily injury liability (found on Declarations page of your policy).
  • Two years of direct support experience in a behavioral health setting is preferred
  • Medication Manager Certification and CPR & First Aid Certifications are preferred, however training will be provided if you are not currently certified.

 


In-Home Services

Personal Assistant, Part-time

Flexible scheduling with hours ranging from 3 – 7 hours per pay period.

Additional pay for 1 or more years work experience in a HCBS &/or college related education for AA or BS/BA.  Shift and weekend differentials also available.

All positions at Hills & Dales require the ability to pass an extensive background check including checks of the Dependent Adult and Child abuse registries, Sex Offender registry and a criminal history check. Offer of employment is contingent upon successful completion of criminal background checks & drug screening. EOE.
