The Hills & Dales Home and Community Based Services (HCBS) program provides families the power and opportunity to use resources available within their community, which allows the family to remain together in the family home. This approach focuses and builds upon each child’s strengths, interests, talents, preferences and targets assistance to achieve specific results in the child’s life. The HCBS program advocates for and supports each child’s awareness and choice, while insuring that their health and safety are maintained. Hills & Dales Home and Community Based Services Programs currently serves 79 individuals and their families within Dubuque and neighboring counties.
